Consular Services

Issuance of Passport

Applications for passport services, may be filed personally by the applicant or by mail. The following are the requirements:

• A duly accomplished application form available at the Consulate General or from this website;
• Old Philippine passport;
• Landing Papers (e.g. Permanent resident card or the old landed immigrant form; employment authorization)
• Any documentary proof as basis for scripting middle name;
• Two passport-size photographs (1.77" x 1.37" or 4.50 cm. x 3.48 cm.) with plain, white background showing front view, without eyeglasses. Photographs of passport applicants wearing sleeveless shirts for men and low-neck attire for women are not acceptable;
• The passport fee is CAD$76.00, payable in cash, money order or certified check.

If application is by mail, the following are additional requirements:

• The passport application form must be duly notarized and
• A self-addressed stamped enveloped (preferably Canada Express post) which the consulate will use in sending back the applicant's new passport.
