Report of Marriage, Birth & Death

Births

For recording of births abroad of children born to Filipino parent(s), the following are required:

1. Personal Apperance
2. Original and three (3) Photocopies of Statement of Live birth (long form) issued by the office of the Civil Registrar General, Canada
3. Duly Accomplished Report of Birth Forms (Triplicate Copies) Fee: C$28.75
4. 3 Copies of Each of the following documentary evidence of Philippine Citizenship of Father or Mother at the time of birth of child:
1. Marriage Contract of Parents (if married)
2. birth certificate
3. Philippine Passport
4. Certificate of Naturalization
5. Three (3) Passport - size photographs
6. Affidavit of illegitimacy (if not married) Fee: C$28.75
7. Triplicate copies of Affidavit of Delayed Registration if registration took place one (1) year after birth. fee: C$28.75

Deaths

For recording deaths of Filipino nationals abroad, the following are required:

• Copy of Death Certificate;
• Passport of the deceased;
• A duly accomplished form (Report of Death of Philippine Citizen) available at the Consulate General
• Fee of CAD$28.75

Marriage

For reporting of marriage solemnized in Canada

• Duly accomplished Report of Marriage Forms (in triplicate copies);
• Original and three (3) photocopies of Marriage License or Registration of Marriage (long form) issued by the Office of the Civil Registrar General;
• Certificate Finality of Nullity of Marriage issued in the Philippines must be authenticated by the Department of Foreign Affairs;
• Three (3) 2x2 photographs each of husband and wife
• Copy of Passport of the couple
• PHOTOCOPY OF THE ABOVEMENTIONED REQUIREMENTS IN TRIPLICATE

Amendment fee of CAD$30.40 and Report of Marriage fee CAD$28.75 payable in cash, money order or certified cheque payable to the Philippine Consulate General.
