Report of Marriage, Birth & Death

Births

For recording of births abroad of children born to Filipino parent(s), the following are required:

• Copy of certificate of live birth (long form);
• Valid passport of parent
• Fee of CAD$38.00

Deaths

For recording deaths of Filipino nationals abroad, the following are required:

• Copy of Death Certificate;
• Passport of the deceased;
• A duly accomplished form (Report of Death of Philippine Citizen) available at the Consulate General
• Fee of CAD$38.00

Marriage

For reporting of marriage solmneized in Canada

• Duly accomplished Report of Marriage Forms (in triplicate copies);
• Original and three (3) photocopies of Marriage License or Registration of Marriage (long form) issued by the Office of the Registrar General;
• Certificate Finality of Nullity of Marriage issued in the Philippines must be authenticated by the Department of Foreign Affairs;
• Three (3) passport-size photographs each of husband and wife (see Item A, No. 5 for specification)
• Copy of spouse's Passport.

Amendment fee of CAD$30.40 and Report of Marriage fee CAD$38.00 payable in cash, money order or certified cheque payable to the Philippine Consulate General.
