Report of Marriage, Report of Birth, Report of Death and Shipment of Remains/Urn

Civil Registry (Report of Birth, Marriage and Death) applications are to be submitted by mail.

BIRTH

For recording of births abroad of children born to Filipino parent(s), the following are required:
  1. Original and four (4) photocopies of Statement of Live Birth / Registration of Birth (long form) issued by the Office of the Registrar General (for Ontario) and Vital Statistics Agency (for Manitoba)
  2. Duly accomplished Report of Birth forms (in four originals, TO BE ACCOMPLISHED IN BLACK INK.) (ROB forms should be duly notarized when reporting by mail) Fee: C$36.25
  3. Applicants can pay in cash or money order/bank draft payable to the Philippine Consulate General in Toronto (personal cheques are not accepted). Alternatively, applicants can opt to pay electronically using credit card, debit card, Apple Pay or Google Pay.

  4. Original and four (4) Copies of Each of the following documentary evidence of Philippine Citizenship of Father and Mother at the time of birth of child:
    • If parents are married, Certificate of Marriage (if married in the Philippines) or Report of Marriage (issued by PSA or Foreign Service Post) or Marriage License (long form) issued by the Office of the Registrar General (for Ontario) or Registration of Marriage (long form) issued by Vital Statistics (for Manitoba)
    • birth certificate
    • Philippine Passport
    • Certificate of Naturalization
  5. One (1) Original and four (4) copies of Affidavit of Delayed Registration of Birth if registration took place one (1) year after birth.(should be duly notarized when reporting by mail) Fee: C$36.25
  6. Applicants can pay in cash or money order/bank draft payable to the Philippine Consulate General in Toronto (personal cheques are not accepted). Alternatively, applicants can opt to pay electronically using credit card, debit card, Apple Pay or Google Pay.

  7. If child is 18 years of age or older, please present the original and four (4) photocopies of the "Negative Certification of Birth" issued by the Philippine Statistics Authority. Please refer to www.psaserbilis.com.ph on how to order online.
  8. For reports of unmarried parents, please execute four (4) copies each of the following Affidavits (should be duly notarized when reporting by mail): (Fee: C$36.25 each)

    Applicants can pay in cash or money order/bank draft payable to the Philippine Consulate General in Toronto (personal cheques are not accepted). Alternatively, applicants can opt to pay electronically using credit card, debit card, Apple Pay or Google Pay.

    1. Affidavit of Admission of Paternity (executed by the father) ;
    2. Affidavit to use Surname of Father(Executed by the Mother/guardian);
    3. Sworn Attestation (Executed by the Mother)
  9. Four (4) copies of Joint Affidavit of Legitimation (for children whose parents subsequently married)(should be duly notarized when reporting by mail) Fee: C$36.25

FOR MAILING OF DOCUMENTS, APPLICANT MUST PROVIDE A SELF-ADDRESSED/STAMPED MAILING ENVELOPE, PREFERABLY AN XPRESSPOST FROM CANADA POST FOR TRACKING PURPOSES. PLEASE INCLUDE YOUR TELEPHONE NUMBER AND/OR EMAIL ADDRESS IN CASE WE NEED TO CONTACT YOU.

The Consular Officer reserves the right to require additional proof or documents from an applicant, to prove his/her citizenship or identity, and ensure accurate and complete personal data entries.

The Civil Registry Unit may require additional documents, if needed.

Processing of Report of Birth takes around five (5) business days upon receiptof the report with complete requirements. This processing time excludes delivery-time for release by mail.


DEATH

MORTUARY CERTIFICATE

(For Shipment of Remains)

LIST OF REQUIREMENTS

The following documents must be submitted

  1. Four (4) photocopies of the deceased’s passport data page;
  2. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) (if death occurred in Ontario), or REGISTRATION OF DEATH (if death occurred in Manitoba) issued by the office of Registrar General;
  3. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) (Ontario and Manitoba) issued by the Office of Registrar General;
  4. One (1) original and three (3) photocopies of EMBALMER’S CERTIFICATE with the declaration that the casket contains only the remains of the deceased, among others (under oath before a Notary Public or person authorized to administer);
  5. One (1) original and three (3) photocopies of MEDICAL HEALTH OFFICER CERTIFICATE;
  6. One (1) original and three (3) photocopies of Certificate for SHIPMENT OF BODY OUTSIDE OF ONTARIO (if death occurred in Ontario) or Certificate for SHIPMENT OF BODY OUT OF PROVINCE (if death occurred in Manitoba);
  7. One (1) original and three (3) photocopies of BEREAVEMENT AUTHORITY OF ONTARIO CERTIFICATE (if death occurred in Ontario), or FUNERAL BOARD OF MANITOBA CERTIFICATE/LETTER (if death occurred in Manitoba); and
  8. Four (4) copies of FLIGHT ITINERARY of the shipment containing the flight details, information of the consignee (name of consignee, complete address, and telephone number) and AIRWAY BILL number.
  • Fee of CAD$36.25 for five (5) working days processing.
  • For expedited processing: Additional fee of CAD$14.50
  • Applicants can pay in cash or money order/bank draft payable to the Philippine Consulate General in Toronto (personal cheques are not accepted). Alternatively, applicants can opt to pay electronically using credit card, debit card, Apple Pay or Google Pay.

Note:

If the deceased is a Filipino national at the time of death, a Report of Death should be filed with the following additional requirements:

  1. Duly accomplished Report of Death Forms . (in four originals, TO BE ACCOMPLISHED IN BLACK INK) signed by the informant for the deceased. (ROD forms should be duly notarized when reporting by mail)
  2. Four (4) photocopies of any proof of the Filipino Citizenship of the deceased (e.g. Passport).
  3. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) issued by the Office of the Registrar General.
  4. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) issued by the Office of the Registrar General.

MORTUARY CERTIFICATE

(For Shipment of Cremated Remains/Ashes)

LIST OF REQUIREMENTS

For SHIPMENT OF CREMATED REMAINS/ASHES properly contained in an urn, the following documents must be submitted:

  1. Four (4) photocopies of the deceased’s passport data page;
  2. One (1) original and three (1) photocopies of STATEMENT OF DEATH (Form 15) (if death occurred in Ontario), or REGISTRATION OF DEATH (if death occurred in Manitoba) issued by the office of Registrar General;
  3. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) (Ontario and Manitoba) issued by the Office of Registrar General;
  4. One (1) original and three (3) photocopies of CERTIFICATE OF CREMATION stating that the urn contains only the ashes of the deceased;
  5. Four (4) photocopies of the passport data page of the person bringing the urn;
  6. Four (4) photocopies of FLIGHT ITINERARY of the person bringing the urn;

Note: Applicant need not bring the urn to the Philippine Consulate for the purpose of applying a Mortuary Certificate.

Fee of CAD$36.25 for five (5) working days processing.

Expedited Processing: Additional Fee of CAD$14.50

If the deceased is a Filipino citizen at the time of death, a Report of Death should be filed with the following additional requirements:

  1. Duly accomplished Report of Death Forms (in four originals, TO BE ACCOMPLISHED IN BLACK INK) signed by the informant for the deceased.
  2. Four (4) photocopies of any proof of the Filipino Citizenship of the deceased (e.g. Passport)
  3. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) issued by the Office of the Registrar General.
  4. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) issued by the Office of the Registrar General.

Report of Death forms should be duly notarized when reporting by mail.

MARRIAGE

For reporting of marriage OF FILIPINO CITIZENS solemnized in Canada, the following are required:
  • Personal appearance of both parties
  • Duly accomplished Report of Marriage (ROM) Forms (in four originals, TO BE ACCOMPLISHED IN BLACK INK.) (ROM forms should be duly notarized when reporting by mail) Fee: C$36.25
    Payable in cash, money order or certified cheque payable to the Philippine Consulate General. Personal cheque is not accepted.
  • Original and four (4) photocopies of Certified Copy of Marriage Registration (long form) issued by the Office of the Civil Registrar General (for Ontario) or Registration of Marriage issued by Vital Statistics (for Manitoba);
  • If applicant had a previous marriage, certificate of Finality of Nullity of Marriage issued in the Philippines must be authenticated by the Department of Foreign Affairs (original and four copies);
  • Original and four (4) photocopies of the data page of the passport of the couple
  • Original and four (4) copies of Affidavit of Delayed Registration of Marriage, if reporting one (1) year after the date of Marriage. (should be duly notarized when reporting by mail) Fee: C$36.25.
  • Payable in cash, money order or certified cheque payable to the Philippine Consulate General. Personal cheque is not accepted.

  • For marriage solemnized more than ten (10) years ago, please submit an original and three (3) photocopies of the “Certificate of No Marriage Record” (CENOMAR) issued by the Philippine Statistics Authority. Please refer to www.psaserbilis.com.ph on how to order online.
PLEASE BE INFORMED THAT, FOR PURPOSES OF RENEWING A PASSPORT, THE APPLICANT WHO WISHES HER NEW PASSPORT TO BEAR HER HUSBAND'S SURNAME SHOULD FIRST OBTAIN FROM THE PHILIPPINE STATISTICS AUTHORITY (PSA) A CERITFIED ROM PRINTED ON PSA SECURITY PAPER.

THE CONSULATE PREPARES TRANSMITTAL OF ROMs TO THE NSO, THROUGH THE DFA, ON A MONTHLY BASIS.

The Civil Registry Unit may require additional documents, if needed.

Fee for Report of Marriage is CAD$36.25. Applicants can pay in cash or money order/bank draft payable to the Philippine Consulate General in Toronto (personal cheques are not accepted). PLEASE NOTE THAT THE CONSULATE GENERAL IN TORONTO CAN ONLY ACCEPT REPORTS OF BIRTHS, MARRIAGES, AND DEATHS THAT OCCURRED IN AREAS UNDER ITS CONSULAR JURISDICTION - MANITOBA AND PARTS OF ONTARIO, WEST OF KINGSTON.
